The Renfrew Victoria Hospital (RVH) is a 55-bed, fully accredited facility located just 45 minutes west of Ottawa, serving a catchment area of over 60,000.  Part of the Renfrew and Area Health Village, RVH provides medical, surgical and complex continuing care as well as extensive ambulatory care with over 30,000 emergency and clinic visits per year.

Come join us! We are recruiting for a Social Worker, Nephrology. This is a permanent, full-time position.

The Social Worker provides service to people at all stages of chronic kidney disease, performing the following duties:

  • Assess the psychosocial needs of nephrology patients and families affected by renal disease and its treatment
  • Provide supportive care pertaining to the emotional and social consequences of medical treatment, dialysis, kidney transplantation, hospitalization, illness and related issues
  • Intervene on a preventative basis with recently diagnosed patients to promote changes in lifestyle and coping mechanisms for optimal health and independence
  • Promote timely access to relevant community resources such as the Kidney Foundation, transportation services, Ontario Health at Home, ODSP, etc.
  • Assist patients in preparing for transplant to get to necessary appointments
  • Act as a liaison between the multidisciplinary team and the patient or family
  • Ensure that all patients have goals of care discussions and documentation
  • Submit data to Ontario Renal Network
  • Assist patients with applications for medication coverage and other financial assistance
Qualifications
  • Current certification with the College of Social Workers and Social Service Workers of Ontario
  • Successful completion of a bachelor's or master’s degree in social work
  • Excellent communication and interpersonal skills
  • Ability to understand, interpret and carry out written and verbal instructions
  • Ability to set priorities
  • Ability to work effectively and provide care within a team environment and contribute to and work with a patient-centered plan of care
  • Ability to work independently and to perform duties within the physical demands of the work area
  • Basic knowledge of established infection control and occupational health and safety practices

Assets:

  • Experience working with people with chronic diseases
  • Bilingualism (English and French)
