We are currently recruiting for a Registered Dietician to work onsite. This is a part-time position working 45 hours per month..

Responsibilities:

  • Identify and monitor the level of nutrition and hydration risk for all residents.
  • Complete scheduled RAI-MDS assessments; assess nutritional status for all residents upon admission, annually, and when significant changes in status occur.
  • Prioritize and respond to dietary referrals in a timely manner.
  • Develop, evaluate, and modify nutritional care plans for all residents; liaise with residents, family members and/or substitute decision-makers and the interdisciplinary team when implementing dietary interventions.
  • Conduct swallowing assessments.
  • Provide nutrition counselling and education to residents and family members and/or substitute decision-makers, as needed, in line with supporting the resident’s nutrition care.
  • Collaborate with the Food and Nutrition Manager to implement a pleasurable dining experience for residents.
  • Participate in care conferences and committee meetings.
  • Review and approve seasonal menus and develop individualized menus for residents.
  • Provide education and training to dietary and nursing staff to support home-specific policies and procedures.
  • Participate in home-specific continuous quality improvement initiatives.
Qualifications

Required:

  • Member in good standing of the College of Dietitians of Ontario
  • Must have liability insurance, TB testing, and a vulnerable sector police check
  • Ability to work in a team and independently
  • Good verbal and written communication skills
  • Excellent organizational and time management skills
  • Demonstrates an ability to problem-solve

Assets:

  • Membership in Dietitians of Canada – Gerontology Network
  • An active member of CSNM
  • Experience in long-term care
  • Computer literacy with knowledge of computerized electronic medical records (EMR) and Point Click Care programs
Additional Information

Valley Manor is offering a generous sign-on bonus for this position. Travel costs such as mileage and accommodation will be covered if required.
