Hospice Renfrew is a six-bed, palliative care home located in the heart of Renfrew. Our exceptional palliative care team provides physical, emotional, and spiritual care to individuals living with a terminal illness and their families.
We are currently seeking a caring and compassionate Registered Social Worker or Social Service Worker to oversee our grief and bereavement program as well as provide psychological support and counselling to our residents and their families in our hospice. This is a full-time position.
Clinical Responsibilities:
- Provide psychological assessment and counselling of patients, their family members, caregivers and clients who are bereaved
- Facilitate monthly support groups (for bereaved and caregivers)
- Attend and participate in interdisciplinary clinical rounds and meetings
- Liaise with community services and refer family members to appropriate community resources
- Provide support to hospice residence staff and volunteers regarding psychosocial issues
- Collaborate with the volunteer coordinator to oversee the grief and bereavement volunteer team
- Provide one-on-one grief counselling to community members who meet the criteria
Administrative Responsibilities:
- Manage and maintain clinical files and related documentation in accordance with the policies and procedure of Hospice Renfrew
- Provide ongoing evaluation of the program
- Participate as a member of a multidisciplinary team
- Participate in short- and long-term planning for the program
